Reporting to Quality and Patient Safety leadership within the hospice and palliative service line, the Hospice Quality Improvement RN will be well-supported by experienced clinical educators, data analysts, and friendly nurses within a diverse, interdisciplinary team that includes physicians, APPs, social work, chaplaincy, pharmacy, PT/OT, and case management. As a Clinical Quality Nurse, Hospice, daily work includes eliciting front-line recommendations for quality and safety, encouraging just-culture occurrence reporting, investigating events, leading root cause analyses, implementing and monitoring patient safety and performance improvement programs, developing and delivering PI education (including leadership development), supporting units in annual PI initiatives, overseeing data collection and chart abstraction, identifying and reporting quality measures, preparing QAPI reports for monthly Quality Outcomes Committee meetings, integrating tracer and survey learnings into action plans, leveraging dashboards to track quality and safety metrics, and serving as a proactive liaison to clinical departments to identify issues, implement solutions, and verify sustainment.
The Hospice Quality Improvement RN requires a current RN license, five or more years of nursing experience with at least two years in quality, patient safety, or performance improvement, and proven exposure to hospice and palliative medicine or related high-acuity settings such as ICU, emergency, or medical-surgical.
